Basic Pediatric Echo Application Review

Updated on

This article reviews the general function of the EncaptureMD pediatric echo application. It is complete with Z scores and Mullens diagrams for a comprehensive pediatric echo report.

Detail

The application has multiple tabs

  • The H&P is an optional tab used to enter a basic patient history and physical
  • Measurements including Z scores are a way to quickly review all imported measurements
  • The Interpretation tab is where the user can enter normal and abnormal findings
  • The Diagrams tab has an anterior and posterior Mullens diagram
  • The Counseling tab provides a template to create patient counseling documentation based on the echocardiogram results.

H&P

  1. The left of the screen allows entry of the standard components of a patient history either through free texting or phrase generation
  2. Vital signs are usually imported from the modality but can also be manually entered here.
  3. Blood pressures (one or multiple) can be entered here and are especially valuable for the assessment of coarctation

Measurements

  1. Imported measurements that have Z scores are displayed here. If the calculations are not present or the ranges are off, the height and weight must be present to properly calculate the Z scores.
  2. There are cases of larger children who exceed the Z score algorithm calculators. In this case, adult ranges can be applied. To do this, the measurements must be transferred out of the Z score calculator to the Adult values section:
    1. Click Copy to Adult
    2. Click Clear Z
  3. Chambers, Vessels and Valves are similar screens that show additional measurements not directly associated with a Z score.

Interpretation

The layout of this screen is very similar to our adult echo application, but follows a more pediatric/congenital approach.

  1. The first column contains cardiac position and general connections plus details about various syndromes. The next section covers valves. Note: if you comment on an AV Canal valve, do not comment on the mitral or tricuspid valve. Similarly, if you comment about a truncal valve, do not enter data under AV or PV.
  2. The middle column is mostly cardiac chambers. LV and RV are "morphologic" indicating they may not actually be in the typical postion. The lower section contains great vessel and other findings.
  3. The right column shows the body and conclusion of the report. Edit by clicking on an existing sentence
  4. The Copy from Previous button will pull the quantitative information (not measurements) from a previous study, if it exists.
  5. The sonographer can create a note for the attending. This note does NOT end up in the final report.
  6. The All Else NWS(not well seen) and All Else Normal buttons set any report section to normal if it does not have any abnormal values. Normal work flow is to fill out all abnormalities then click the All Else Norm button at the end.

Diagrams

An anterior and/or posterior diagram can be added to the report.

  1. Choose the starting diagram from the combo box
  2. Use the tools as needed to sketch or add labels to the diagramd

Counseling

  1. Create one or more templates and choose the one that makes sense for the clinical scenario
  2. When the text for the template appears here, edit accordingly.
